• Daily huddles with care team to review schedules, patients, resources and coordination of care.
• Pharmacists assist with patient education and medication management.
• Care managers and clinic staff work collaboratively with providers.
• Increased access to care via eVisits, patient portals and ConvenientCare clinics.
