Job summary

Geisinger Mt Pocono in northeastern PA is seeking a new graduate or experienced Nurse Practitioner or Physician Assistant to support our family medicine team in Mt Pocono, PA. The role offers a schedule of Monday–Friday 8am to 4:30pm with one late night per week and a Saturday rotation, including weekend call rotation.

In this position you will work with a care team, utilize eVisits and patient portals, and engage with pharmacists, care managers, and clinic health staff to improve access to care

Qualifications

• Treat patients of all ages (birth to geriatric).
• Master’s degree in PA or NP; PA required, NP preferred.
• Licensed/certified (NCCPA and BLS) per state requirements.
• Strong care management and documentation skills.

Responsibilities

• Provide extended 40-minute visits for complex patients; 20-minute visits for average/acute cases.
• Schedule: Mon–Fri 8:00–4:30; one late night weekly and Saturday rotation.
• Participate in daily care team huddles to coordinate care.
• Collaborate with pharmacists, care managers, and clinic health staff.
• Support eVisits, patient portals, urgent care access, and messaging.

Job Summary

Geisinger Mt Pocono in northeastern PA, is seeking a new graduate or experienced Nurse Practitioner or Physician Assistant to support our family medicine team in the beautiful area of Mt Pocono, PA.

Job Duties

• Medically complex patients receive extended forty minute appointments; twenty-minute appointments for patients with average complexity and acute care visits.
• Schedule: Monday through Friday 8am to 4:30; including one late night per week and Saturday rotation.
• Call: Rotation includes rotation of weekend call.
• Daily huddles with all care team members to review essential information about schedules, patients, resource needs and coordination of care.
• Pharmacists to assist with patient education and medication management.
• Availability of care managers, health managers, clinic health associates and coding educators to providers.
• Increased access to care by means of eVisits, patient portals, Geisinger’s ConvenientCare urgent care clinics and even text messaging.
