Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Blue Hill, NE. The PTA will provide services under the Physical Therapist and help implement the plan of care.

Responsibilities include screenings, treatment support, progress documentation, and collaboration with the therapy team. Part-time or PRN positions with competitive pay and opportunities for growth.

Qualifications

• Assesses residents and plans therapy with oversight from Physical Therapist.
• Maintains licensure and participates in continuing education.
• Collaborates with the therapy team to deliver care.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation and treatment planning with short- and long-term goals.
• Perform individual and group treatments per treatment plan.
• Monitor resident response and modify as directed by the PT.
• Assist in scheduling in coordination with the PT and team.
• Train residents and staff in equipment use and safety.
• Document in NetHealth EMR as required.

Education

Associate's degree in physical therapy from an accredited program

Active Physical Therapy Assistant license in state(s) of practice

Ongoing continuing education and professional development
