Concept Rehab & Engage Consulting in Adrian, Michigan is seeking a licensed Physical Therapist (PT) for a part-time role at Lynwood Skilled Nursing Facility. You will evaluate residents, develop treatment plans, and support mobility and independence.

We value compassionate, independent clinicians who collaborate with an interdisciplinary team. Michigan PT license in good standing and degree from an accredited program are required; prior geriatric/post-acute experience is preferred.

Qualifications

• Licensed Physical Therapist in Michigan (in good standing).
• Degree in Physical Therapy from an accredited program.
• Experience in skilled nursing, post-acute rehab, or geriatric care preferred.
• Ability to work independently while contributing to a multidisciplinary team.
• Previous experience in skilled nursing or geriatric care is preferred.

Responsibilities

• Evaluate residents’ functional abilities, mobility, balance, strength and rehab needs.
• Develop individualized treatment plans based on assessments and goals.
• Provide interventions to improve walking, transfers, coordination and daily function.
• Monitor progress and adjust treatment approaches as needed.
• Document evaluations, sessions, progress notes and discharge recommendations.
• Collaborate with nursing, OT, PT, SLP and other team members.
• Educate residents, families and caregivers on exercises and safety strategies.
• Follow established clinical standards and documentation requirements.
