Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care, in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition to supervising staff.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs and informs patient and family regarding plan of care, limitations, and progress.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes patient evaluations and develops PT plan within state guidelines.
• Reports outcomes and anticipated frequency of care.
