Job summary

Continuum Therapy Partners is seeking Physical Therapists to join a therapist-led team and support a growing national footprint. The role emphasizes evaluating and implementing individualized treatment plans in collaboration with other disciplines.

Candidates must hold a Doctor of Physical Therapy (DPT), current WI license, and NPTE eligibility; 1 year of PT experience is preferred. Part-time daytime positions are available at Meadowview Health Services and Plymouth Health Services in Sheboygan

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
