Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Minot, ND. This part-time/PRN role focuses on planning, directing and delivering high-quality PT services to residents in accordance with CMS and local standards.

Responsibilities include evaluation, treatment planning, documentation, and collaboration with the interdisciplinary team to ensure optimal resident care and outcomes. Active license and ongoing CE courses are required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per established standards.
• Document and bill for therapy services in CMS/NetHealth systems.
• Develop and implement treatment plans with short- and long-term goals.
• Evaluate residents and communicate findings to families and team.
• Screen residents regularly to determine intervention needs.
• Complete compliant, medically necessary documentation.
