Job summary

Evolve Therapy Services seeks a Physical Therapist for an in-house Skilled Nursing Facility in Minot, ND. This Part-Time or PRN role focuses on delivering high-quality PT services to residents in a dedicated facility.

You will plan, implement, and document therapy programs, collaborate with the care team, and ensure CMS-compliant records. Active licensure in the state and ongoing CE are required for continued eligibility.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents according to standards and procedures.
• Adhere to documentation and billing procedures for PT services.
• Develop and implement short- and long-term treatment plans.
• Conduct screening and evaluations to determine intervention needs.
• Communicate evaluation findings and recommendations to residents and the inter-disciplinary team.
