Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Minot, ND. This part-time or PRN role focuses on delivering high-quality therapy services to residents, conducting assessments, and coordinating with an interdisciplinary team.

Responsibilities include developing treatment plans, documenting per CMS standards in NetHealth EMR, and providing ongoing professional development to maintain licensure.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice.
• Adhere to procedures regarding documentation and billing of physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Develop treatment plans by establishing short and long-term goals and methods.
• Interpret and communicate evaluation findings and recommendations to residents and the interdisciplinary team.
• Complete compliant and medically necessary documentation per CMS expectations in NetHealth EMR systems.
