HealthPro Heritage is seeking a part-time Physical Therapist at our assisted living community in Holmdel, NJ. Join a therapist-led, collaborative team focused on quality care, mobility, and patient education. This role offers a consistent weekday schedule with no evenings or weekends required.

You will assess patients, design and implement treatment plans, document progress, and collaborate with other healthcare professionals to achieve optimal outcomes.

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Valid state licensure as a Physical Therapist, or license eligible.
• Strong clinical assessment and diagnostic skills; able to develop and implement effective treatment plans.
• Excellent communication and interpersonal skills with a patient-centered approach.

Responsibilities

• Patient assessment: Evaluate patients' physical conditions, mobility, strength and flexibility.
• Treatment and education: Provide therapeutic exercises, manual therapy, and education for home routines.
• Documentation: Maintain accurate records of evaluations, progress, and plans.
• Collaboration: Work with healthcare providers and participate in multidisciplinary meetings as needed.
