Job summary

Adventist HealthCare Home Health Rockville is seeking a part-time Physical Therapist (PT) to join our team in Rockville, MD. The role involves performing initial examinations, planning and implementing rehab plans, and coordinating with the care team to ensure patient progress.

The ideal candidate holds a Maryland PT license (Bachelor/Master/Doctorate), BLS certification, and prefers 1 year of site-specific experience. Weekend rotation and a valid driver's license are expected as part of duties.

Qualifications

• Bachelors, Masters, or Doctorate in Physical Therapy from accredited physical therapy program.
• 1-year site specific preferred
• Active Maryland License
• Valid driver's license with agency insurance requirements
• Active American Heart Association Basic Life Support (BLS) certification required

Responsibilities

• Perform an initial examination and evaluation to establish impairment/diagnosis, prognosis and plan of care for rehabilitation needs.
• Provide, or direct and supervise, therapy intervention consistent with examination and plan of care.
• Plan for discharge considering goals and outcomes, with appropriate follow-up or referral.
• Communicate, coordinate and document all aspects of patient management with the care team.
• Manage schedule to accommodate census and caseload; inform management of issues.
• Attend team meetings and communicate patient status.
• Be responsible for individual professional development activities.
