Job summary

Freeman Home Care is seeking a Part-Time Physical Therapist to join the Freeman Health System in Joplin, MO. You will independently evaluate patients, develop goal-directed treatment plans, deliver and document care, and assist with discharge planning for physician-referred cases.

Qualifications include current Physical Therapy licensure in Missouri, at least two years of PT experience, a clean driving record, auto liability insurance, and the ability to pass safety and background checks.

Qualifications

• Current Physical Therapy licensure in state of practice (Missouri).
• At least two years of experience as a physical therapist.
• Auto liability insurance required.
• Clean driving record and valid driver's license.
• Must register with and pass the Family Care Safety Registry upon hire.
• Able to pass KDADS criminal record check.
• AHA BLS for Healthcare Providers within 90 days of hire.
• Continual Education: AHA BLS for Healthcare Providers required within 90 days.

Responsibilities

• Independently evaluates patients, develops goal-directed treatment plans.
• Delivers and documents care, and provides discharge planning for referred patients.
• Supervises and directs the treatment and activity of support staff.
• Coordinates daily workflow and scheduling in the Home Health therapy department when Lead PT is absent.
