Job summary

AW Health Care in St Peters, MO is seeking a Part Time or PRN Physical Therapist to support our Independent/Assisted Living partnerships. The role involves providing intermittent PT services in patients’ homes or care facilities under physician orders and plan of care.

Key duties include patient assessments, PT evaluations, planning and implementing treatment, supervising PTA, and coordinating with the interdisciplinary team.

Qualifications

• Graduate of an accredited PT program with a bachelor degree.
• Current MO license and national certification as Physical Therapist.
• Two years of experience preferred.

Responsibilities

• Assesses patient functional ability and potential.
• Completes PT evaluation to determine benefit and plan of treatment.
• Develops and implements medically prescribed PT plan of treatment.
• Supervises PTA and evaluates progress at least every 30 days.
• Documents each visit timely and communicates with the team.
