Job summary

CommuniCare Rehab in Point Pleasant, WV, is seeking a Part-Time Physical Therapist to join Pleasant Valley Healthcare Center. You will evaluate residents, develop and implement treatment plans, and collaborate with an in-house rehab team to deliver patient-centered care.

This role offers flexible part-time scheduling, opportunities to work across multiple centers, and ongoing CEU resources. A PT license is required or pending, with degrees from an accredited program.

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
