Job summary

QuadMed is seeking a Part-time Physical Therapist to join Kohl's Wellness Center in Pataskala, OH. The role offers 20 hours per week with full scheduling flexibility within health center hours.

Responsibilities include evaluation, treatment, care planning, patient education, discharge planning, and accurate billing. Requires a PT degree with CPR/BLS; 1 year of experience minimum, orthopedic experience preferred.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program; Doctorate preferred.
• Minimum 1 year of experience; 3 years orthopedic clinic experience preferred.
• Current CPR (BLS) certification or must be obtained within 60 days of hire.

Responsibilities

• Evaluates and treats patients.
• Interfaces with patient's MD regarding care and progress; refers to appropriate provider as required by law.
• Establishes plans of care and implements evidence-based treatment based on evaluation results and goals.
• Counsels and educates patients on injury, illness, treatments, and wellness.
• Completes discharge planning including home program design and patient/family education.
• Documents and codes billing accurately to maximize reimbursement.
• Maintains cleanliness and organization in the clinic; adheres to safety standards.
• Oversees students and interns; assists with training as needed.
• Prepares monthly and annual reports.
• Performs other duties as assigned.
