Qualifications

• Minimum Bachelor's Degree in Physical Therapy from an accredited university or college; Doctorate in Physical Therapy preferred.

Responsibilities

• Evaluates and treats patients in outpatient setting.
• Interfaces with physician regarding care and progress; direct access referrals as required by law.
• Develops and implements a plan of care with functional goals; updates provider team.
• Counsels patients on injury, illness, treatment and wellness.
• Discharges patients and designs home program; provides teaching to patient/family.
