Job summary

Tracy Nursing and Rehabilitation Center in Tracy, CA is seeking a licensed Physical Therapist for a Part-Time role with a clear path to Full-Time as the program grows. You will assess residents, develop individualized treatment plans, and help restore mobility and independence in a collaborative skilled nursing setting.

You will guide a team, supervise PTAs, and ensure compliant documentation while contributing to program development and resident outcomes in a supportive, mission-driven

Qualifications

• Active California Physical Therapist license required.
• Graduate of an accredited PT program (Bachelor's, Master's, or Doctorate).
• NPTE certification completed or in good standing.
• Current CPR/BLS certification or willingness to obtain.
• Experience in skilled nursing or post-acute settings preferred.

Responsibilities

• Perform comprehensive evaluations and reassessments per physician orders and state practice.
• Develop, implement, and adjust individualized plans of care with measurable goals.
• Deliver skilled PT interventions, individually and in group settings.
• Monitor progress and modify treatment based on outcomes.
• Supervise PTAs and rehab staff per California law.
• Document timely, accurate records meeting regulatory requirements.
• Participate in care conferences, discharge planning, and quality initiatives.
