Knowledge Builders Inc. is seeking a licensed Physical Therapist to perform initial evaluations and develop individualized treatment plans for newly admitted patients and those returning from outside hospitalization.

The position is 20 hours per week, Monday to Friday, with a flexible schedule. Requirements include Doctor of Physical Therapy (DPT) registered by NYSED or CAPTE, NPTE passage, and NYS licensure; minimum one year experience.

Qualifications

• DPT from a program registered by NYSED or CAPTE, or equivalent.
• Pass the NPTE and hold NYS licensure to practice PT.
• Minimum of one year of clinical experience.

Responsabilités

• Perform initial evaluation and functional assessment of patients.
• Develop individualized treatment plans based on findings.
• Provide skilled physical therapy interventions and monitor progress.
• Document evaluations, treatment, progress notes and discharge summaries in the EHR.
• Educate patients and direct care staff on diagnosis, plans, prevention strategies.
• Collaborate with physicians and other team members for coordinated care.
• Serve on Safe Patient Handling and Falls committees as needed.
