Job summary

Santa Rosa Post Acute in Santa Rosa, CA is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team in a part-time, in-person role. You will evaluate residents, develop individualized treatment plans, and provide evidence-based therapy to improve mobility and independence.

A $5,000 sign-on bonus is available, with a staged payout after 45 and 90 days of continuous employment. You will work with an interdisciplinary team in a skilled nursing and post-acute care setting,

Qualifications

• Current California Physical Therapist (PT) license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively within an interdisciplinary healthcare team.
• SNF / Post-Acute care experience preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide evidence-based physical therapy services to improve mobility, strength, balance, and functional independence.
• Assess patient needs and establish measurable treatment goals.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, therapy team members, and families.
• Educate patients and caregivers regarding treatment plans, exercises, and safety recommendations.
• Participate in interdisciplinary care planning meetings and patient conferences.
• Supervise PTAs as appropriate.
• Recommend adaptive equipment and mobility aids when necessary.
• Maintain accurate and timely documentation per guidelines.
