Western Michigan University Homer Stryker M.D. School of Medicine is hiring a Part-Time Patient Navigation Assistant in Kalamazoo, MI. The role supports the Street Medicine Kalamazoo team to improve access to medical care for unhoused populations.

The position involves patient advocacy, data collection, and coordination with care teams, with benefits including tuition reimbursement and multiple health plan options.

Competenze

• High School Diploma or GED is required.
• Currently enrolled in an undergraduate or graduate program.
• Interest in healthcare and serving the unhoused population.
• Willingness to learn Epic and Cerner EHR systems.

Mansioni

• Assist with patient navigation tasks and support patients in acquiring basic needs.
• Deliver basic supplies to patients including food, medications, clothing, tents, tarps, and other essentials.
• Act as patient advocate by accompanying patients to appointments and assisting with calls.
• Participate in weekly encampment outreach and document patient interactions in the medical record.
• Assist with data collection around patient navigation and needs; attend weekly care-team meetings.
