Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license to transport individuals in agency vehicles.
• Ability to pass background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain certifications.
• Commitment to a safe, healthy, and professional work environment.
• Willingness to stay beyond scheduled shifts to ensure coverage.

Responsibilities

• Support individuals in living healthy, meaningful, and fulfilling lives.
• Assist with personal care, grooming, bathing, dressing, and hygiene needs.
• Provide continence care as needed.
• Promote independence and skill development.
• Monitor health information and document vitals when required.
• Administer medications per training and maintain records.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and snacks according to dietary needs and preferences.
• Assist with mobility and transfers using approved techniques and equipment.
• Maintain accurate documentation in the EHR and follow regulatory requirements.
• Communicate with team members, individuals served, and families; participate in trainings.
