PREMIER THERAPY is seeking a Part-Time Occupational Therapist (OTR) for Lakewood, Ohio. This role focuses on skilled rehabilitation in a skilled nursing setting, with a compassionate, resident-centered approach and a collaborative care team.

Responsibilities include evaluating residents, creating individualized treatment plans, and delivering evidence-based therapies to improve daily living skills, while maintaining thorough documentation and safety.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Ohio Occupational Therapist license or eligibility for licensure.
• Strong clinical judgment, communication, and documentation skills.
• A compassionate, dependable, and resident-focused approach to care.
• Ability to work independently while collaborating with an interdisciplinary team.
• Previous skilled nursing or geriatric rehabilitation experience preferred, but not required.

Responsibilities

• Evaluate residents to identify functional limitations, strengths, and rehabilitation needs.
• Develop and implement individualized, goal-driven occupational therapy treatment plans.
• Provide evidence-based interventions to improve independence with activities of daily living.
• Recommend adaptive equipment and compensatory strategies to promote safety and functional performance.
• Monitor resident progress and adjust treatment plans based on clinical findings and outcomes.
• Collaborate with physical therapists, speech-language pathologists, nursing staff, and other healthcare professionals.
• Educate residents, families, and caregivers on strategies that promote independence and safety.
• Maintain accurate and timely clinical documentation in accordance with facility policies and applicable regulations.
• Deliver compassionate, resident-centered care that supports meaningful rehabilitation outcomes.

What You’ll Do as an Occupational Therapist

As a Part-Time Occupational Therapist, you’ll provide skilled occupational therapy services designed to help residents improve their functional abilities and overall quality of life.

Your responsibilities will include:
