Stern at Home Therapy is hiring a part-time Occupational Therapist (OT) to provide one-on-one home health care for geriatric patients in North Buffalo and Cheektowaga, NY. The role offers a consistent part-time caseload with flexible scheduling and strong clinical autonomy.

Responsibilities include evaluations, developing plans of care, and promoting independence in daily activities. Ideal candidates have NY OT licensure and reliable transportation for home visits.

Qualifications

• Active Occupational Therapist license in New York is required.
• Graduate of an accredited Occupational Therapy program.
• Reliable transportation is required for home visits.
• Strong clinical, communication, documentation, and time‑management skills.

Responsabilités

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide one‑on‑one occupational therapy treatments in patients’ homes.
• Help patients improve independence with activities of daily living.
• Address upper‑extremity strength, ROM, cognition, safety, and functional performance.
• Provide therapeutic activities and exercises based on patient goals.
• Educate patients and caregivers on adaptive strategies and safety.
• Monitor progress and modify treatment plans as needed.
• Complete accurate clinical documentation and communicate with care team.
