Qualifications

Responsibilities

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient’s needs and goals.
• Manage treatment plans with clinical autonomy.
• Help patients improve independence, safety, mobility, and activities of daily living.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, facility staff and the clinical team.
• Coordinate with administrative team regarding scheduling, patient care and logistics.

Stern at Home Therapy is hiring an Occupational Therapist to provide one-on-one care to patients in their homes and at an established facility in the Ransomville area.

Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability.

This opportunity is ideal for a licensed Occupational Therapist looking to build a steady part-time caseload or add flexible PRN visits to an existing schedule, treating one patient at a time with support from an experienced administrative and clinical team.

What You’ll Do:

As an Occupational Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population throughout the Ransomville and Niagara Falls areas.

Your caseload may include both home health visits and treatments at our established facility in Ransomville, providing variety while minimizing unnecessary travel.

Responsibilities include:

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient's functional needs and goals.
• Manage individualized treatment plans with clinical autonomy.
• Help patients improve independence, safety, mobility, and their ability to perform daily activities.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, facility staff, and members of the clinical team.
• Coordinate with our administrative team regarding scheduling, patient care, and logistics.
• Manage your caseload based on your availability and patient needs.

Compensation:

Part‑Time W‑2:

• $50-55 per follow-up visit.
• $65 per 4-unit evaluation.

PRN:

• $62 per 4-unit follow-up visit.
• $75 per evaluation.
