Job summary

Stern at Home Therapy in Manhattan is seeking Occupational Therapists (OTs) for full-time, part-time, and PRN roles to deliver one-on-one home-based care across the East and West Sides.

You will evaluate and treat geriatric patients in their homes, build individualized plans of care, and collaborate with an administrative team to coordinate referrals, scheduling, and documentation.

Qualifications

• Evaluate and treat a geriatric population in patients' homes and develop individualized plans of care.
• Provide one-on-one occupational therapy services with a focus on independence, safety, and daily living activities.
• Complete accurate clinical documentation and communicate with physicians, caregivers, families.

Responsibilities

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide one-on-one occupational therapy treatments in patients' homes.
• Help patients improve independence with activities of daily living.
• Address strength, range of motion, cognition, safety, and functional performance.
• Provide therapeutic activities and exercises based on each patient's goals.
• Recommend adaptive equipment and home modifications.
• Educate patients and caregivers on strategies, exercises, and safety.
• Monitor progress and modify treatment plans.
• Complete accurate and timely clinical documentation.
• Communicate with physicians, caregivers, families, and interdisciplinary care team members.
