Job summary

Yale New Haven Health seeks a part-time Oncology Extended Care APP in New Haven, CT. The role spans 24 hours per week via an interdisciplinary oncology team and involves chemotherapy prescribing, patient assessment, and coordinated care with physicians under Epic EMR.

Qualifications

  • Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
  • 1-2 years of Oncology experience preferred.
  • Active, unrestricted CT medical license or in process of obtaining.
  • BLS Certification as required.
  • DEA License.

Responsibilities

  • Provide assessment, triage, and therapeutic management of patient/family needs as part of an interdisciplinary team throughout the disease process.
  • Collaborate with attending supervising/collaborating physicians in accordance with state law and hospital policies; may provide autonomous team-based care.
  • Obtain medical histories, perform exams, order tests, review results, and formulate treatment plans including chemotherapy prescriptions.
  • Educate patients and families on care, treatment plans, side effects, and discharge plans in collaboration with the team.
  • Provide psychosocial support to patients and families in a caring, ethical, and professional manner.

Responsibilities
  • Provide assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team.
  • Work in collaboration with the attending supervising/collaborating physician(s) in accordance with state law and hospital regulations, hospital policy, protocols, and clinical pathways, and may provide autonomous team‑based care.
  • Obtain medical histories, perform physical examinations, review / order diagnostic / therapeutic tests and radiologic results, chemotherapy prescribing, formulate and manage patient treatment plans.
  • Educate patients and families on care and available resources, in collaboration with the interdisciplinary team. This includes admitting instructions, treatment plans, potential side effects, and discharge plans.
  • As part of an interdisciplinary team, provide psychosocial support to patients, their family members, and significant others in a caring, ethical, and professional manner.
Qualifications

EDUCATION

  • Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.

EXPERIENCE

  • 1-2 years of Oncology experience preferred.
  • Active, unrestricted CT medical license or in process of obtaining.
  • BLS (as required) Certification.
  • DEA License.
