Job summary

Yale New Haven Health is seeking a Part-Time Oncology Extended Care APP in New Haven, CT. The role supports an interdisciplinary team within Smilow Cancer Hospital and affiliated clinics, with a focus on patient-centered cancer care.

Responsibilities include assessment, treatment planning, and patient education across the disease course, using Epic for documentation and care coordination. A CT license and BLS are required, with DEA licensure preferred.

Qualifications

• Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
• Active, unrestricted CT medical license or in process of obtaining.
• BLS (as required) Certification
• DEA License

Responsibilities

• Provide assessment, triage, and therapeutic management of patient/family needs throughout the disease process as part of an interdisciplinary team.
• Collaborate with the attending supervising/collaborating physician in accordance with state law and hospital policies, and may provide autonomous team-based care.
• Obtain medical histories, perform examinations, review tests and radiologic results, prescribe chemotherapy, and manage treatment plans.
• Educate patients and families on care and available resources, including admission instructions, treatment plans, side effects, and discharge plans.
• Provide psychosocial support to patients and families in a caring, ethical manner.

Responsibilities

• As a valued member of our team, you will:
• Provide assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team.
• Work in collaboration with the attending supervising/collaborating physician(s) in accordance with state law and hospital regulations, hospital policy, protocols, and clinical pathways, and may provide autonomous team-based care.
• Obtains medical histories, performs physical examinations, reviews/orders diagnostic/therapeutic tests and radiologic results, chemotherapy prescribing, formulates and manages patient treatment plans.
• The APP will educate patients and families on care and available resources, in collaboration with the interdisciplinary team. This includes admitting instructions, treatment plans, potential side effects, and discharge plans.
• The APP, as part of a interdisciplinary team, provides psychosocial support to patients, their family members, and significant others in a caring, ethical, and professional manner.

Qualifications

EDUCATION

• Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.

Experience

• 1-2 years of Oncology experience preferred
• Active, unrestricted CT medical license or in process of obtaining
• BLS (as required) Certification
• DEA License
