Job summary

Dignity Home Health & Hospice is seeking an On-Call Registered Nurse (RN) to provide after-hours clinical support to patients and families across Utah County. This part-time, on-call role emphasizes independent nursing assessments, timely care, and collaboration with the care team.

The RN will assess patients, coordinate care, document thoroughly, and respond to after-hours needs in homes and facilities. Utah RN license, CPR, and reliable transportation are required.

Qualifications

• 1 year of home health and/or hospice nursing experience preferred.
• Strong assessment skills with sound clinical judgment and critical thinking.
• Calm, dependable, professional approach for urgent or unfamiliar situations.
• Compassion and respect for patients and families.
• Excellent organization, attention to detail, and follow-through.
• Strong written and verbal communication skills.
• Comfort with electronic medical records and healthcare technology.
• Reliable transportation for travel throughout the service area.

Responsibilities

• Respond to patient needs outside regular hours via telephone triage and in-person visits.
• Complete comprehensive nursing assessments in homes, assisted living, and skilled nursing facilities.
• Provide clinical guidance, education, reassurance, and support to patients, families, caregivers, and staff.
• Complete admissions during after-hours when necessary.
• Maintain accurate, thorough, and timely clinical documentation.
• Communicate with the interdisciplinary care team to ensure continuity of care.
