Job summary

Reliant Rehabilitation is seeking an Occupational Therapist to deliver OT services in assisted living, independent living, and memory care settings. You will screen, evaluate, and plan care, supervise COTAs, and collaborate with an interdisciplinary team to ensure high-quality patient outcomes.

The role requires clinical leadership, adherence to OBRA and company policies, and willingness to travel between communities. NBCOT certification and state OT license are required where applicable.

Qualifications

• Graduate of ACOTE-accredited OT program.
• NBCOT certification and state OT license where applicable.
• Proficient in MS Office and basic internet navigation.
• Strong communication and teamwork abilities.
• Ability to travel between communities as needed.

Responsibilities

• Provide OT services including screening, evaluation, and treatment planning.
• Supervise COTAs and rehab techs; coordinate with interdisciplinary team.
• Ensure billing and documentation comply with state/federal guidelines.
• Develop and implement OT programs within OBRA and Reliant policies.
• Travel between communities as required.
