Job summary

Trinity Health in Fresno seeks a licensed Occupational Therapist for part-time Home Health Services. The OT will assess patients, create individualized plans, educate families, and coordinate with an interdisciplinary team. Supervisory responsibilities include guiding COTAs and HHAs and ensuring thorough documentation.

Qualified candidates have NBCOT eligibility or certification and a state license, with 1 year of clinical experience preferred. A $5,000 sign-on bonus is offered.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Eligible for or currently registered with NBCOT.
• Licensed to practice as an OT in the employment state.
• Minimum of one year of clinical experience or 6–8 week preceptorship.
• Home health experience preferred.
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage a flexible schedule.
• Commitment to ethical practice and the mission of St. Agnes Home Health.

Responsibilities

• Conduces comprehensive assessments and develops individualized treatment plans with physician and client.
• Implements therapeutic interventions to improve independence.
• Educates clients and caregivers on adaptive techniques and assistive devices.
• Evaluates home environments and suggests safety modifications.
• Coordinates care with interdisciplinary team and makes referrals as needed.
• Supervises COTAs and HHAs, giving guidance and feedback.
• Maintains accurate documentation in the EHR system.
• Participates in quality improvement, case conferences, and staff education.
• Ensures compliance with federal, state, and agency regulations.
