Coastside Adult Day Health Center in Half Moon Bay, CA is seeking a part-time Occupational Therapist to assess participants, build plans of care, and deliver therapy across daily activities.

Qualifikationen

• Graduate of accredited OT curriculum; registered or eligible for registration.
• Excellent people skills and ability to work in a team-based care model.
• Good work record, reliable, neat, and compassionate toward the elderly and disabled.

Aufgaben

• Initial and continuing assessment of participants and initial treatment plan.
• Re-assessment of participants as conditions change; adjust goals.
• Deliver person-centered care; plan of care addressing needs and abilities.
• Provide input to the multidisciplinary team for collaborative planning.
• Complete TAR/Individualized Plan of Care every 6 months with progress notes.
• Complete quarterly assessments every 3 months with progress notes.
• Complete and sign daily flow sheets indicating therapy received and responses.
• Increase and maintain participants’ independence and aging in place.
• Enhance emotional and social well-being of participants.
• Develop participants’ use of therapeutic activities to improve function.
• Guide participants in using therapeutic activities to improve function.
• Suggest supplies and equipment needed for OT.
• Provide input regarding OT policies and procedures.
• Other duties as assigned.
