Trumen Physicians + Associates is seeking an experienced and compassionate Nurse Practitioner to join our expanding post-acute care team in Tyler, TX. Independent Contractor (1099) position – Practice with autonomy and tax flexibility
The Nurse Practitioner will provide in-person care to patients in post-acute settings, collaborating closely with Trumen physicians and facility staff. Ideal candidates are proactive, compassionate, and skilled in managing chronic and acute conditions within a skilled nursing facility environment.
Conduct in-person assessments and manage patient care in skilled nursing and rehabilitation facilities
Collaborate with Trumen medical directors and interdisciplinary facility teams
Participate in value-based care initiatives and care transitions
Accurately document clinical encounters and care plans in the EMR
Build relationships with facility administrators, nursing staff, and Trumen leadership
Be present in facilities during scheduled hours
Active Texas Nurse Practitioner license
Minimum of 1 year NP experience (post-acute, SNF, or geriatric care preferred)
Strong interpersonal and clinical decision-making skills
Comfort working independently and collaborating as part of a care team
Reliable transportation within the Tyler area
