Trumen Physicians + Associates is seeking an experienced and compassionate Nurse Practitioner to join our expanding post-acute care team in Tyler, TX. About the Role:

The Nurse Practitioner will provide in-person care to patients in post-acute settings, collaborating closely with Trumen physicians and facility staff. Ideal candidates are proactive, compassionate, and skilled in managing chronic and acute conditions within a skilled nursing facility environment.

Core Responsibilities:

• Conduct in-person assessments and manage patient care in skilled nursing and rehabilitation facilities

• Collaborate with Trumen medical directors and interdisciplinary facility teams

• Participate in value-based care initiatives and care transitions

• Accurately document clinical encounters and care plans in the EMR

• Build relationships with facility administrators, nursing staff, and Trumen leadership

• Be present in facilities during scheduled hours

Qualifications:

• Active Texas Nurse Practitioner license

• Minimum of 1 year NP experience (post-acute, SNF, or geriatric care preferred)

• Strong interpersonal and clinical decision-making skills

• Comfort working independently and collaborating as part of a care team

• Reliable transportation within the Tyler area
