Job summary

CareMed Health Solutions is seeking Nurse Practitioners to provide primary and preventive medical services to residents living in long-term care facilities. You will work with an interdisciplinary team to deliver comprehensive care, including managing acute changes, advanced care planning, and chronic disease management.

Responsibilities include prescribing medications per OH law, ordering and following up on tests, and coordinating with PCPs.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification required.
• DEA licensure required.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus
