Job summary

Integrated Wound Care in Wisconsin is seeking a compassionate Nurse Practitioner to join our team on a part-time basis. You will provide focused wound care to residents in skilled nursing, long-term care, and rehabilitation facilities, aiming to reduce hospitalizations and improve healing outcomes.

You will perform wound assessments, develop individualized treatment plans, document meticulously, and educate patients and families on prevention and management.

Qualifications

• Active license in state of employment.
• Minimum 1 year of wound care experience.
• Ability to work independently and as part of a team; strong communication.
• Comfortable with technology and EMR systems.
