Integrated Wound Care is seeking a compassionate Nurse Practitioner to provide focused wound care on a part-time basis. The NP will assess wounds, create individualized treatment plans, and work closely with a multidisciplinary team to improve resident outcomes at facilities served.

The role emphasizes thorough documentation, patient and family education, and ongoing program participation to reduce wound-related hospitalizations and enhance care quality.

Qualifications

• Active license in the state of employment.
• Minimum of 1 year of wound care experience.
• Ability to work independently and as part of a team.
• Excellent verbal and written communication skills.
• Strong time management and organizational abilities.
• Familiarity with electronic medical records (EMR).

Responsibilities
