Job summary

Institute on Aging in Merced, CA seeks a Part-time Nurse Liaison to educate and train RCFE partners, serve as content expert for waived services, and collaborate with the Health Plan team to ensure quality measures are completed.

The role provides eight hours of support per week, requires California RN license, and a minimum of two years’ experience with disabled or older adults. Strong communication, training delivery, and computer literacy are essential.

Qualifications

• CA RN license required; 2+ years with disabled/older adults.
• Experience delivering trainings to diverse groups.
• Excellent verbal and written communication skills.
• Experience with health plans or SNF/Hospital settings preferred.

Responsibilities

• Act as consultant for Care Management staff regarding medical health issues.
• Participate in multidisciplinary team meetings and care conferences; provide recommendations.
• Ensure smooth transitions from institutional settings to community living with supports.
• Review discharge paperwork and consult with care providers as needed.
• Provide ongoing trainings to CalAIM staff relevant to client population.
• Complete assessments and documentation for waived services programs.
• Monitor utilization patterns and assist with health promotion interventions.
• Attend team meetings and contribute to CQI efforts.
