Gateway Medical in the Greer/Greenville, SC area is seeking an experienced contract/part-time Nurse Practitioner to provide primary care within an assisted living setting. Typical day includes 8–15 visits across one facility, with physician support as needed.

Qualifications

• Licensed NP in South Carolina.
• Board certification in AGPCNP/AGACNP or Family Nurse Practitioner.
• Previous primary care experience preferred.

Responsibilities

• Diagnose, treat, and manage acute and chronic illnesses.
• Perform health assessments and develop individualized treatment plans.
• Routinely conduct Medicare Annual Wellness Visits and follow-ups.
• Educate patients and families on health promotion and disease prevention.
• Collaborate with facility staff to optimize patient outcomes.
• Prescribe medications and therapies as appropriate.
• Maintain accurate EMR documentation and patient charts.
• Participate in ongoing continuing education and team leadership.
