Job summary

A healthcare provider is seeking a Nurse Practitioner/Physician Assistant to deliver primary care services to patients in an assisted living facility in Tryon, NC. The successful candidate will work closely with a medical director and other healthcare professionals, conducting assessments, developing care plans, prescribing treatments, and providing essential patient education. Previous experience in primary care and relevant licensure are required.

Qualifications

• Current licensure as a Nurse Practitioner or Physician Assistant in the state of Kentucky.
• Master's degree in Nursing or Physician Assistant Studies.
• Certification as a Nurse Practitioner or Physician Assistant.
• Experience in primary care, preferably in an assisted living or long-term care setting.

Responsibilities

• Conduct initial and ongoing assessments of patients' health status.
• Develop and implement care plans based on patients' needs and goals.
• Prescribe medications and treatments as appropriate.
• Order and interpret diagnostic tests.
• Provide patient education and counseling on health promotion and disease prevention.
• Collaborate with other healthcare professionals to ensure coordinated and comprehensive care.
• Maintain accurate and up-to-date medical records.
• Participate in quality improvement initiatives.
