TeamHealth seeks a quality-driven nurse practitioner or physician assistant to join our post-acute care team in Linden, Michigan. This part-time opportunity (1 day per week) offers autonomy, solid support, and earning potential in a fee-for-service model with potential to grow beyond base.

You will work in skilled nursing facilities, collaborate with an experienced multidisciplinary team, and provide high-quality care while building meaningful relationships with patients and families.

Formación

• NP or PA license (Michigan) and DEA registration.
• Experience in post-acute, acute, ED, or clinic settings preferred; new grads welcome.
• Strong clinical foundation and knowledge of healthcare regulatory standards.
• Excellent communication and interpersonal skills; collaborative team player.

Responsabilidades

• Perform comprehensive patient assessments, including medical histories and physical exams.
• Develop, implement, and adjust individualized care plans based on patient needs.
• Monitor progress and coordinate care to optimize outcomes.
• Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach.
• Educate patients and families on health management and preventive strategies.
• Maintain accurate, timely medical records in compliance with regulations.
