Job summary

TeamHealth is seeking a quality-driven nurse practitioner or physician assistant to join our post-acute care team in Syracuse, New York. This is a PRN opportunity with 2-4 weekend days per month, rounding in skilled nursing facilities, with strong support and training.

You'll work with an experienced multidisciplinary team, enjoy autonomy, and have earning potential through fee-for-service with no cap on productivity income.

Qualifications

• Post-acute, acute, ED, or clinic experience preferred; new grads welcome to apply.
• Excellent communication and interpersonal skills.
• Collaborative, team-oriented approach with a positive outlook.
• NY NP or PA license and DEA registration.

Responsibilities

• Perform comprehensive patient assessments, including medical histories and physical exams.
• Develop, implement, and adjust individualized care plans based on patient needs.
• Monitor progress and coordinate care to optimize outcomes.
• Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach.
• Educate patients and families on health management and preventive strategies.
• Maintain accurate, timely medical records in compliance with regulations.
