Job summary

Esse Health is seeking a Nurse Practitioner or Physician Assistant for our Fenton Family Medicine office at 714 Gravois Rd. The position is part-time (24 hours weekly), three days a week, outpatient primary care for adults, with collaboration alongside a physician partner.

Ideal candidates have 3+ years of NP/PA experience, Missouri licensure, and strong clinical and communication skills in a team-based setting. EOE

Qualifications

• Master’s Degree of Science in Nursing or Physician Assistant Master’s Degree.
• Current MO NP or PA licensure (or eligibility).
• Certified NP or PA (or eligibility).
• Strong clinical, communication, and interpersonal skills.
• Commitment to patient-centered, team-based care.
• Ability to work independently while collaborating with physicians.

Responsibilities

• Provide comprehensive primary care services to adult patients.
• Conduct patient evaluations, physical exams, and diagnostic assessments.
• Diagnose and manage acute and chronic conditions.
• Develop and implement treatment plans in collaboration with physician partner.
• Emphasize preventive care, wellness, and patient education.
• Document patient care accurately in the electronic health record (NextGen).
• Coordinate care with specialists and other healthcare team members.
• Participate in quality improvement and care coordination initiatives.
