Esse Health is seeking a compassionate Advanced Practice Provider (NP/PA) for the Hazelwood office. The role offers full-time or part-time scheduling with regular business hours and outpatient focus in internal medicine.

Ideal candidates hold Missouri NP/PA licensure, a master's degree, and strong communication skills. The practice emphasizes patient-centered, team-based care and integrated electronic records (NextGen).

Qualifications

• Master’s Degree of Science in Nursing or Physician Assistant Master’s Degree.
• Current NP or PA licensure in Missouri (or eligibility).
• Certified NP or PA (or eligibility).

Responsibilities

• Join a collaborative team and develop a 1:1 relationship with patients and clinical staff.
• Provide comprehensive primary care services to adult patients.
• Conduct patient evaluations, physical exams, and diagnostic assessments.
• Diagnose and manage acute and chronic conditions.
• Develop and implement treatment plans with physician partner.
• Emphasize preventive care, wellness, and patient education.
• Document care accurately in the electronic health record (NextGen).
• Coordinate care with specialists and other team members.
• Participate in quality improvement and care coordination initiatives.
