Qualifications

Responsibilities

• Assess physical, social, and psychological status of patients and gather information from the care team.
• Develop and implement an individualized plan of care with patient and family involvement.
• Coordinate care with interdisciplinary team and manage time, supplies, and resources.
• Provide or delegate care per plan and ensure timely documentation and compliance.
• Evaluate patient response and revise the care plan as needed; uphold professional standards.
