Qualifications

• Weekend and holiday coverage required.
• Knowledge of Microsoft Office products and keyboarding.

Responsibilities

• Leads patient assessment and complex discharge planning.
• Collaborates with physicians and healthcare team to plan, implement and evaluate care plans.
• Facilitates coordination of care and interventions for identified needs.
• Serves as discharge planning/resource for psychosocially complex cases.
