Back to You Osteopractic Physical Therapy & Rehabilitation/Home and Back to You is seeking a part-time driver to transport patients in the Metro Detroit area, primarily Rochester and Royal Oak. You will drive to appointments, maintain logs, and ensure documentation is filed in patient charts.

The role requires a clean driving record, a chauffeur license, and a DOT card, plus the ability to pass a health physical and drug test.

Qualifications

• Requirement to have a clean driving record, a chauffeur license, and DOT card.
• Health physical and drug test may be required.
• MVR Consent Form may be required.

Responsibilities

• Drive patients door-to-door to and from appointments as scheduled by transportation manager.
• Maintain driver transportation logs and upload documentation to patient chart.
