Responsibilities

• Serve as the attending physician for patients at assigned SNFs.
• Collaborate with NPs/PAs to manage daily patient care and provide clinical supervision.
• Deliver high-quality, patient-centered primary care, diagnosing and treating acute and chronic conditions.
• Participate in chronic care management, transitional care, remote patient monitoring, and other quality initiatives.
