Job summary

Northwest Passage is seeking a part-time Medical Coordinator to support nursing and medical coordination across our sites in Wisconsin. The role involves coordinating care, administering medications, documenting health information, and communicating with clinicians.

The position requires a Wisconsin LPN license or Medical Assistant certification, strong documentation skills, independence, and teamwork. Approximately one day per week, with potential coverage as needed.

Qualifications

• Current Wisconsin license as a Licensed Practical Nurse or certification as a Medical Assistant.

Responsibilities

• Provide nursing and/or medical coordination services at assigned sites.
• Establish and maintain effective working relationships with healthcare providers involved in the care of residential clients.
• Administer and document prescribed medications in accordance with provider orders and agency policies.
• Collect, document, and monitor medical information and concerns during intake and throughout treatment.
• Coordinate and schedule medical appointments, including acute care visits, laboratory services, intake appointments, and follow-up care.
• Maintain supplies of medications and organize medication kits.
• Participate in case management and interdisciplinary meetings as needed.
• Complete medical discharge summaries and related documentation.
