Qualifications

• Current California LVN license in good standing.
• Experience in skilled nursing or long-term care preferred.
• Working knowledge of MDS 3.0 and RAI processes.
• Familiarity with CMS/state/facility reimbursement guidelines.

Responsibilities

• Coordinate and complete MDS assessments and CAAs within required timelines.
• Ensure documentation reflects resident conditions and supports reimbursement.
• Maintain CMS/state/facility regulatory compliance.
• Collaborate with interdisciplinary team to update individualized care plans.
• Monitor resident progress and update care plans as needed.
• Assist during audits, surveys, and regulatory reviews.
• Educate staff on MDS documentation standards.
