Job summary

San Bruno Skilled Nursing is seeking a knowledgeable MDS Nurse (LVN) to join our team on a part-time basis. You will manage MDS assessments, CAAs, and ensure regulatory compliance to support high-quality resident care.

Ideal candidates hold a California LVN license, have experience with MDS 3.0 and reimbursement guidelines, and can collaborate with an interdisciplinary team to maintain accurate documentation and timely submissions.

Qualifications

Responsibilities

• Coordinate and complete MDS assessments and CAAs within timelines.
• Ensure CMS/state/facility regulatory compliance in documentation.
• Collaborate with interdisciplinary team on individualized care plans.
• Monitor progress and update care plans as needed.
• Support audits, surveys, and regulatory reviews.
